Excursions
If you have a great idea for a school excursion and you have the support of a staff member who is willing to take on the role of lead teacher, here are the steps to follow:
The lead teacher completes and submits a Principal's authorization form (511A) along with a Parent/Guardian permission form (511C);
Once the 511A and 511C are approved, the lead teacher distributes copies of the 511C to students;
Signed 511Cs are collected by the lead teacher, copies are left with main office staff (along with a list of participating students), and originals are brought on the trip.
Notes:
The most important information on the 511Cs is the health card number and the emergency contact number;
More complex excursions involving overnight stays or volunteer drivers require additional forms to be completed.
